About Dementia

Dementia is not a specific condition; it's a term that represents a range of symptoms, including memory loss. In order to be diagnosed with dementia, a patient needs to have at least two of the following symptoms: memory loss, communication and language impairment, inability to focus, problems with vision and/or ability to reason.

About Alzheimer's

Alzheimer's is the most common form of dementia. This disease presents itself in memory loss and changes to behavior, as well as how a patient thinks. Alzheimer's is a progressive disease where symptoms worsen over time.

Services

Memory Screening: Our licensed clinical social worker provides memory screenings by appointment and no referral is needed. We share results of a memory screening with caregivers and primary care physicians, as appropriate, to keep their whole care team informed and up-to-date.

Call 901-227-1234 to make an appointment. No doctor’s referral is necessary for the screening.

Resource Networking: Our social workers can help guests and their caregivers find resources for local home care, respite care, day care, driving safety, community events, emotional guidance and other related needs.

Support Services: As our guests and their caregivers go through emotional and everyday life changes, our staff provides caregiving classes, community outreach services, support groups and advanced care planning.
